For 2026, your out-of-pocket spending on covered Part D drugs is capped at $2,100, the maximum deductible is $615, and there is no coverage gap. These changes come from the Inflation Reduction Act, which also lets you spread drug costs across the year with the Medicare Prescription Payment Plan. The trade-off: many plans raised premiums or deductibles, so the plan that was cheapest last year may not be this year.

How the cap works
What you pay for covered Part D drugs counts toward the cap: your deductible, copays and coinsurance, plus payments made on your behalf by Extra Help and most manufacturer assistance. Plan premiums do not count. Once you reach $2,100 you pay nothing for covered Part D drugs for the rest of the calendar year. Drugs given in a doctor’s office under Part B do not count.
The Medicare Prescription Payment Plan
Instead of paying a large pharmacy bill in January, you can opt in to have your plan bill you monthly for your drug costs, spread across the remaining months of the year, with no interest. It does not lower the total; it levels it. It helps most for people on one expensive medication early in the year.
Why premiums and deductibles moved
Capping what you pay shifts cost to plans, so insurers have raised premiums, added deductibles up to $615, and moved some drugs to higher tiers. That is why we rerun every client’s medication list each fall: a plan that was cheapest in 2025 can be far from cheapest in 2026.
